Raleigh Plastic Surgery Center is committed to maintaining the privacy and security of the information that it maintains. Raleigh Plastic Surgery Center recently identified a data security incident impacting the personal information of a limited number of Raleigh Plastic Surgery Center patients.

Upon learning of incident, Raleigh Plastic Surgery Center immediately launched an investigation and took steps to mitigate against any harm. Raleigh Plastic Surgery Center’s investigation determined that on April 30, 2022, a former employee deposited certain records containing personal and protected health information in a dumpster located in an apartment complex. The employee’s action was witnessed by another individual, who promptly recovered the records from the dumpster and notified the U.S. Department of Health and Human Services Office for Civil Rights, which subsequently notified Raleigh Plastic Surgery of this incident on January 17, 2023.

As a result of this incident, Raleigh Plastic Surgery Center determined that certain files and folders deposited in the dumpster may have contained identifiable personal and/or protected health information for a very limited number of Raleigh Plastic Surgery Center patients, including the individuals’ full name and one or more of the following: medical treatment and diagnosis information, treatment costs, dates of service, prescription information, Medicare or Medicaid number, and/or health insurance information. This incident does not affect all patients of Raleigh Plastic Surgery Center and Raleigh Plastic Surgery Center does not necessarily maintain all of the information listed above for all impacted individuals.

Raleigh Plastic Surgery Center has no evidence to suggest that any information has been used or misused by the former employee or the individual who recovered the records. However, to date, Raleigh Plastic Surgery Center has been unable to recover the records from the individual who recovered the records from the dumpster and therefore has been unable to conclusively identify any impacted individuals. Any individuals who believe their information was included in the impacted records should review the explanation of benefits statements that they receive from their health insurance providers and follow up on any items not recognized. At this time, Raleigh Plastic Surgery Center has no reason to believe any Social Security numbers were included in the impacted files.

The security of the personal information in its possession is Raleigh Plastic Surgery Center’s top priority. In response to this incident, Raleigh Plastic Surgery Center has reviewed its existing policies and procedures to determine if improvements could be made and has also required its workforce members to complete additional privacy and security training.
